Enroll online by following these Three Steps:

REMINDER: ALL THREE MUST BE COMPLETED TO HAVE A SUCCESSFUL ENROLLMENT!!!

1. Complete the Online Enrollment Application.
   
   
2. Complete the Online PHLOTE Form.
   
   
3. Complete the Online McKinney-Vento Form.

Once you have enrolled the student, please make an appointment to visit your Sequoia Choice site and turn in any applicable documentation that may be missing such as:

• Birth Certificate
• Immunization Records (State requirements [PDF])
• Transcripts
• Withdrawal Slips
